For some reason, reality has changed in some way to make diapers a bit more conducive for Vanilla. But what kind of shift could have caused this?

The first option is that, for some reason, Cream and Vanilla have swapped roles. Despite Vanilla being the older rabbit and Cream's mother, the younger bunny is suddenly the one acting motherly, doting on Vanilla as though she was a young child.
