Patrick's Arch

Patrick wriggles his foot around in his shoe a little and you fall in. He has his laces loose so as he twists his foot you fall straight down under the arch of his foot.
You need to find something to hold on to. You end up finding a hole in his sock and a tare in the base of the shoe under his arch. How will you secure yourself?
