The disability is with Jane's Grandfather, who has dementia and is the reason Jane is stuck in the small town and unable to travel as she dreams to. Her love for him causes her to table that dream as long as he's well enough to stay with her.
The mental illness I guess is her ex-friend / renewed friend Averil, who had serious depression and self esteem issues.
They are both side characters but I expect Averil's story will be the main one in the next book and the Grandfather had lots of scenes, some even without Jane. And he was instrumental in helping Jane consider Kurt as more than what she originally intended.
