Both.

Before I write anything, I turn it around in my mind. Sometimes it escapes from there and I never see it again. Other times, I eventually start writing down ideas or at least general notes.

When it comes to novel-length stuff, I plot the big stuff and pants the details. For short stories, I generally just start writing, knowing where I want it to go.
