The only options for informing readers of the presence of lesser rated content is to add the lesser content rating to the entry title by hand or to make a statement about it to go along with the link. This is strictly informational and does not count as an actual content rating because it cannot be corrected by mods+ if necessary.

Thus, if the entry content exceeds the content rating for the whole book, the Book's content rating must (not should) be changed to acknowledge that higher rated content.
