If you choose the Novel subtype AND the TOC: Chapters option in the Item Specific section, the book is supposed to add new entries to the end of the TOC lists with the default 0 for the ordering number of all the entries.
As I recall, this has been a known bug for a while. The manual reordering, or the custom format, are currently the workaround solutions to the problem.
Creating a dummy book with nearly blank entries (to fill in as you write them) and then using the manual reorder once, numbering from the bottom with single/double digits (it'll "hundred" them for you) may be the most convenient way to deal with it. The nearly blank entries won't show to others if they're set to private until you fill them in for real.
