There's different ways. Sometimes, there's a phrase in the novel that stands out and would make a good title. Other times, there's a theme in the novel that can be summed up in a short phrase for the title.

Typically, my titles have come from brainstorming with my writing group. For the first novel, the title came from an informal gab session with two of them. For the second novel, they had expressed some concerns that one of the words I was using in the title was too academic, so, while spending a weekend with one of my writing partners, we brainstormed something up that meant the same thing, captured the same feel that I was after, but with a more accessible meaning.
