Where you find markets depends on what you're attempting to submit.
Writer's Market is a good, all-purpose place to start. They have an annual print version that can be a bit pricey, depending which one you select. The local library often keeps a print copy in stock. They also have an online edition, which is inexpensive and stays more up-to-date than the print version. Both aren't always as up-to-date as I might like. The print version also has model copies of query and cover letters.

You can also go to the bookstore and browse the publications that you like and think your work might fit in with. Most individual publishers have a website with guidelines available.
