For the planning problem, have you considered setting a goal each day, where you either write x-number of words (they don't have to be good words) or write for y-number of hours? You could also try to find a writing group and set up a schedule for exchanging work. No exceptions, you must turn in some writing to them by a given time.
It really is just a matter of making yourself write, whether you are inspired or not. You just have to move forward.
