Personally, I have one rule: Structure is for rewrites
Draft until you run out of steam, then go back, pick out the threads you want to emphasize, create an outline, flesh out the characters with full descriptions, then rewrite the key passages, rewrite the support passages, and then go through the whole manuscript from beginning to end adding polish.
'Course, I haven't published anything worth mentioning...
