If she's not up for that, then she should rethink this whole idea of being a published novelist. That's what a really good book does - provokes reaction. I don't think Stephen King, Patricia Cornwell, or any number of other authors got where they are today by playing coy when it came to the grisly details. Not all readers enjoy it (some loathe even mild profanity - I say let them write their kind of novel, let me write mine), but I think if you write with honesty and it's integral to the story, anything goes. If you just throw those elements in for the sake of sensationalism, readers and publishers will see right through it.
