That would depend on how often one swears. If you use 10 swear words in a 1,000 word item, you'd be fine to rate it 13+ (excluding the one word mention - which tends to get a higher rating anyways). But, if that same-sized item has 100 swear words - I'd make that an 18+ at the bare minimum, but it would probably be safer to just make it a GC at that point.
