On the Survey Form's creation page under Section 1: Basic Item Information, choose the correct "WebSite" on which the message forum should be displayed.
Be sure to choose "Allow Anonymous Submissions" and "Allow Multiple Submissions" so that any visitor to your website can fill out your form. If you only want your form for members of your website's community, do not choose those options.
To reference the Survey Form within your website, type: /view/###### (replacing the ###### with the survey's item ID.
If your website has a Header and a Footer configured for it, the Survey Form will show up automatically between the two using your site's standard colors and settings.
