It means something or someone is completely ok, well, or healthy again, especially in the case of recovering from an illness or a bad experience. Charles Dickens's Pickwick Papers uses a different form of this expression: "right as a trivet." It wouldn't surprise me if "right as rain" is the grandchild of that expression since, in England in those times, Dickens's work had infected common speech.
