As a musician, I am going to eat this up. I apologize.
Decent writing, and it's nice and short so I don't have to rack my brain too hard. I'm confused about the structure, though. Does each section correspond to each movement of the Moonlight Sonata? Or are you only considering the first movement? If the former case, then the second section doesn't seem to link up. At least to me. The second movement never really conveyed to me ideas of hopelessness and things "worse that dying." On the other hand, the first and third sections work pretty well. And if you were only considering the first movement, and I'm just wasting brainpower here, then that works pretty well, too.
Just seems like the metaphorical connections between the poem and the music are a bit confused. But perhaps you have inspired me to think about the Moonlight Sonata in a different light.
