They ignored the signs until one day as they tried to log out and take the headset off they found that they couldn't feel anything. The NPC had grown a sense of AI and every time they put the helmet on the npc switched into them. At first the npc needed the helmet on but as the character used it more and more the npc synced with their body until they finally took control leaving the character as the npc.
#2 Rehabilitation program- A new program had been implemented. It was a chance for criminals to get out of jail for a day or two with good behavior. Except they would be swapped with someone that made it so they couldn't hurt anyone if they wanted to. Except there was a problem with one of the swaps. When it came time to swap back between a criminal and a volunteer the machine misfunctioned. Though was it a accidental problem or did the criminal prepare it?
#3 A Spy Mishap- There was a new unit of spies, ones that volunteered their bodies literally. These spies would infiltrate areas by swapping with people, though this was done by a machine that the spy organization had. So when (Insert name here) swapped with (Insert another name here) to do a infiltration mission they didn't expect to come back to the person in their body not being kept under surveillance but instead out free? Little did they know that the swap was erased from the records and now the spy was on the run from their own agency and their own body!
#4 Exchange Island Mess Up- When (Insert character name) went on vacation with their friend to exchange island it was amazing. They enjoyed the stay and the swap and were a bit disappointed to go back. But when the swap back happended they didn't end up in their own body but ended up in someone elses? They tried to tell someone but everyone just brushed them off as they watched in panic as their own body went home with their friends.
