Here are some more ideas for a tiny boy in a fantasy setting.

Every Party Needs a Healer: The tiny boy is a healer for a group of female adventurers, who is very dutiful in his work.

The Demon Queen’s Son: The boy is the son of the Demon Queen, who rules over a dark realm and is attended to by many demonic maidens. At some point, his mother will kidnap a princess to wed him, which will possibly conflict with the a similarity tiny prince or Knight.

Fairy Tale: The boy is born a fairy, a tiny race of humans with wings, who lives hidden in plain sight amongst the humans. In this route, the fairy boy is discovered by a peasant, a bandit, or a member of a high social hierarchy like nobility or royalty. What happens to the fairy boy is up to you.
