Confess it all and trust Sarah to understand

You decide that the best policy is to be honest and hold Sarah's hand while she was mumbling something. Then as Sarah stirred awake, you take a deep breath and say "Honey, I know this may be a bit much, but the other day, I met this lady called Trish," Before she could protest you gently shush her as you continue talking, "One thing led to another and before you knew it, you were unconscious. I had consulted the doctors and they said that it was okay as Trish was a viable donor and I needed to save your life, so they did the next best thing, which was transferring your mind into Trish's body." You gave a rather apprehensive and nervous smile and hoped this information stuck into Sarah's mind.
