The rules: The first card dealt must be face up. It determines the swap. Anyone who doesn't fold during a hand is immune to swapping. Any player that folds, loses a trait to the player on the right.
The face up card determines the trait lost in the following way: Aces mean intellectual ability. Kings mean hair. Queens mean eyes. Jacks mean complexion. Tens mean genitals. Nines mean torso. Eights mean butt. Sevens mean arms. Sixes mean legs. Fives mean nose. Fours mean ears. Threes mean mouth. Two means personality (including fashion sense, interests and hobbies). Suit determines duration. Clubs mean an hour. Hearts mean a day. Diamonds mean a week. And spades mean a month.
I will let you decide if swaps have to match or not. With matching swaps, a player swaps traits for the same traits such as legs for legs. With unmatching swaps, legs can be swapped for arms however, aces, jacks, nines, and twos cannot be swapped except for matching traits. So unmatching swap can easily become complicated. I'll leave it up to you to decide how you want to handle the complications.
