Dr. Moriarty was going to switch their chests and their crotches between them, but he had a more funny thought. As he looked at Amanda and Joseph he laughed at the thought of a grown woman complaining about her breasts, and her young son saying he had them.
The same went for Stephen and Kristen.
Everything went right with the switch, and now Joseph remembered himself as 10 year old boy with D-cup breasts, a mature vagina and being pregnant.

Stephen had on his daughter's pj shirt and shorts with her girly panties underneath, with a penis clearly noticeable from underneath.
Kristen had on her father pj's with boxers underneath clearly to large for her.
Dr. Moriarty didn't adjust their clothings because he wanted to see their reactions to their old clothing, which was the other switch made by Moriarty.
