The pressure of a ticking clock doesn't work for everyone! And I'm sorry about your arthritis. That is NO FUN. This might be more trouble than it's worth, but have you considered dictation software? I find it's a pain to go back, re-read and fix errors; it's easier just to type it and get it mostly right in the first place (yes, I make typos, but at least entire words aren't incorrect!!) But in your case, if typing physically hurts, it might be worth it.
I have the Evernote app on my Android phone, and it has a speech-to-text conversion. So I can dictate into Evernote and save the "note," then copy/paste the text into my working novel document.
