I know from experience like you that being the only one to understand and love someone with a disorder is not an easy task.
My son is Autistic and not everyone sees him the same way I do - they don't see past the surface to the wonderful little boy. It breaks my heart at times, but I keep putting one foot in front of the other.
Always tell your brother that you love him. Just because we 'know' that we are loved doesn't mean that hearing it diminishes it in some way. Sometimes hearing it is the one thing we need to keep fighting.
Don't let the others get you down. Stand by your brother - he needs you - and hold your head high. Count yourself as one of the few and be proud.
